Medical Forms

I’ve added more than two dozen new forms, charts, and trackers to FreePrintableMedicalForms.com. Some are intended for medical professionals while others are appropriate for use by patients and caregivers.

New medical office templates to aid in patient communication include: a cancellation call list, a courtesy call list, and a checkup reminder call list.

I’ve also added several trackers, such as: a digestion distress tracker, a discontinued medications tracker, and a cold and flu tracker. New medication logs are designed for daily, weekly, or monthly use. There’s also a master health diary with daily, weekly, and monthly variations.

FreePrintableMedicalForms.com now has several physical therapy forms, a caretaker shift form, a drug accountability form, a chemotherapy schedule, a medical encounters form, and forms related to durable medical equipment.

New to the veterinary forms category is a rescue animal transfer form.

The site also has exercise and fitness forms, forms for people with diabetes, family planning forms, nutrition forms, wallet-sized forms and much more. There are nearly 600 forms at the site altogether.

Download and print anything at FreePrintableMedicalForms.com free as a PDF and then write on it in pen or pencil. A customizable DOC (Microsoft Word) version is $3.99 apiece. For medical offices, or anyone who anticipates using several forms and charts, there’s the time- and money-saving option of an all-in-one-download of the medical forms collection for $47.

Contracts

With the addition of 24 new contract templates, there are now 489 contracts at PrintableContracts.com. They’re all free to download, customize, and print.

The new professional services contracts have to do with construction: unit price, time and materials, lump sum, and cost plus.

I’ve added new personal relationships contracts such as: new parent agreement, screen time contract, household rules agreement, and two agreements that cover household chores.

There’s a fresh batch of wedding contracts, too. You’ll find contracts relating to: wedding venue, catering, DJs, videography, transportation, a photo booth, officiant, hotel reservations, hair and makeup, flowers, dress, cake, and musicians.

Other new contracts include an authorization for minor to travel and an affiliate agreement.

The contracts at PrintableContracts.com download instantly as editable DOC files. Or, cut and paste the text directly from the site and format it in a document of your choice. These contract templates are great, but they are no substitute for legal advice, so be sure to consult an attorney when you need one.

Popular Printables

This week, I took a look at what’s most popular among the 800-plus planner refills, lists, and templates at PrintablePlanners.net.

This week’s most-downloaded item is a daily planner with 15-minute intervals. It’s followed in popularity by a colorful daily schedule with blocks for info.

Also popular is a project planner, “cute” five-day planner, a bill tracker, a daily spending log, a password tracker with room for security questions, a 90-day calendar, a business agenda, a two-page monthly planner, a vertical weekly planner, and a planner with room for “tomorrow.”

The site’s five-year journal, which has a few lines to write on for each day of the year, is among the Top 15 most-downloaded items. (PrintablePlanners.net also has a 10-year journal variation.)

I started PrintablePlanners.net a decade ago with sets of planner refills sized for popular commercial day planners such as Day Runner, Franklin Covey, and Day-Timer. The collections have expanded to 10 sizes, including scrapbook-style memory planner dimensions.

Each planner page is free to download and print as a PDF. Some are also available as editable DOC files to open and type into using Microsoft Word. The PDF planner refill collections are $9 per size as a convenient all-in-one download, or you can download and print them one at a time for free.
