The Wacky Edition
It’s been wacky weather week here in Portland, Oregon. First the rain, then the thunder and lighting, then the serious hail. It’s exciting, but I’m ready for some sunshine.

This week, I added a slew of new medical charts, journals, charts, and logs to my medical forms site, FreePrintableMedicalForms.com. There are new printables for medical officers, caregivers, schools, parents… even veterinary forms. The site has more than 500 forms in total thanks to the new additions.

Medical Forms
I’ve added a huge batch of new medical form printables to FreePrintableMedicalForms.com. There are 40 new charts, forms, journals and more. At this site, which complements the forms selection at BusinessFormTemplate.com, you’ll find professional forms for medical offices, trackers for home use, school nurse printables, and too many other printables to list.

The new medical charts include: an anesthesia record, blood count results, daily vitals flow sheet, a fluid balance chart, a medication flow sheet, and a patient injury tracker.

There are new journals and logs such as a controlled substance usage log, an elimination diet tracker, a food reintroduction symptom tracker, a medication reaction tracker, and a nebulizer treatment log. For people with Type 1 or Type 2 diabetes, there’s a daily glucose flow sheet, a control chart, an insulin pump log, and weekly and monthly medication trackers.

I’ve added new exercise and weight loss charts, including a colorful annual weight loss tracker and weekly and monthly trackers.

There are new wallet cards alerting people to conditions such as aphasia, dementia, and epilepsy. (This category also has allergy alert cards and cards with general medical and medication information.)

You’ll also find: an alcohol questionnaire, an allergy action plan, an asthma action plan, birth plans, a first aid treatment form, medication forms, a medication incident report, a pre-employment physical form, and a school medication administration log.

You can download and print anything at the site free as a PDF you can write on in pen or pencil. If you’d like to customize the form, a DOC (Microsoft Word) version is available for $3.99 apiece. If you expect to use several of the forms, the $47 all-in-one-download of the medical forms collection is a great deal.
