Wow! FreePrintableMedicalForms.com, which I started with just a few basic forms back in 2009, has grown to 709 medical forms, charts, journals, trackers, and other printables for all kinds of health-related needs. There are items for medical offices, school nurses, households, and pretty much any other
setting you can think of. (As always, if you need a form the site doesn’t have, please write in and suggest it!).
There are 24 new printables in all. The just-added trackers and logs include: vaccination logs, insurance claims
and copay trackers, and an INR anticoagulation record for people on blood thinners. Also new is a certificate of head lice treatment.
There are also two new variations of menstrual cycle diaries or logs. These are
ideal for anyone who prefers paper tracking, or is concerned about digital records being accessed by others.
I’ve added forms for caregivers, such as an emergency treatment or hospital admission tracker. There’s also a
caregiver contacts list and a medical contacts list.
There are new letter templates at FreePrintableMedicalForms.com addressing: a prescription request, a college dorm accommodation request, a college insurance plan waiver, a medical bill late payment request, and a complaint template for cases when a pharmacist wrongly refuses to fill a prescription.
Inspired by real life, I’ve also
added a pet vomit tracker to help narrow down who’s barfing when, in case veterinary care needs to be sought. Also new to the category is an emergency vet contact list. You can also print bold, red window signs that let rescuers know there are pets inside in case of fire or
another emergency. Another new addition is red tags you can laminate and attach to a dog’s harness letting passersby know that he or she is a nervous rescue pet and needs space.
The site also has wallet cards, exercise charts, nutrition forms, logs for people with diabetes,
medication forms, and more. There are also forms that are relevant during the coronavirus pandemic.
